Why Businesses Fail Their EHO Inspection

The factors that EHOs assess

Mistake #1: 🙈 Unaware Of What The Food Hygiene Inspection Involves

To many business owners, meeting the requirements of the food hygiene rating scheme is like a black box.  The path to follow is not visible – and the rules seem unclear.  Nevertheless, failing to understand how your business will be assessed under the Food Hygiene Rating Scheme (and knowledge of the rules) will be your demise.

To be sure you achieve success, you need to learn how the EHO will assess your business.  And with this in mind, get to know how food hygiene scores are calculated. (**We’ll cover this in a future blog).

Specifically, there are three key areas to focus on (see image above).

Food safety and hygiene procedures: How hygienically food is handled, prepared, cooked, cooled, re-heated and stored (knowledge and behaviour)

Structural requirements: The condition and structure of your premises (it includes cleanliness, layout, lighting, ventilation and pests)

Confidence in management procedures: How you manage and record what is done to make food safe, and whether staff are trained for their role.

These requirements are set out in the Food Standard Agency’s National Food Hygiene Brand Standard.

Your food hygiene rating will be based on an assessment of compliance in those three key areas shown above. To get the highest rating, you must do well in all three parts of the assessment.

Mistake #2:💥 Unaware of How To Fail the EHO Inspection

Business owners are unaware of the common mistakes that lead to failure:

🚫 Failing to take the rules seriously — this is a common pitfall.
🚫 Assuming “we’ve never poisoned anyone” does not mean food is safe.

🚫 Ignoring early signs of pests, which can lead to infestations.

Mistake #3: Starting With The Wrong Mindset

Once you’ve prepped your mind to be in a good place, you are ready to jump into Phase 1 – the Get Ready Phase.  

We’ll be tackling your paperwork first.

1️⃣ Get Ready Phase

In the Get Ready Phase, you’ll undertake a review of your food safety documentation.

Here, your Food Safety Management System (FSMS) is the core of your food safety defences.  The paperwork is there to offer a layer of protection.

To do its job properly, your food safety documentation must not be filed away gathering dust.  Or locked in an office.

In practice, it must be in use on a daily basis.  For example, the EHO will assess your documentation to see whether it holds up under scrutiny.

💡So, here are some questions to ask yourself during your food safety document review.

Food Safety Document Review

Does your Food Safety Management System (FSMS) reflect your business activities?

Specific Questions To Ask

❓Does your Food Safety Management System outline clear tasks, roles, and responsibilities?

❓Do staff know how to prevent contamination?

❓Have you trained staff on those procedures?

❓Are staff following your safe operating procedures?

❓Do they know why each step matters?

❓Have you documented staff training?  And are your records up-to-date?

❓Has monitoring of processes like cooking, chilling and hot holding been documented?

❓Did you write down corrective action when problems occurred?

❓Are there any unexplained gaps in your records?

❓Did a manager review and sign off all monitoring records?

When To Review Your Food Safety Management System (FSMS)

 Ask yourself:

 Has your documentation been kept up to date?

More specifically:

➊ Can you show that your Food Safety Management System has been reviewed and updated in the past 12 months. (You need to do this sooner, if significant changes have occurred).  

👉Find out When to review your HACCP Plan.

➋ Does your FSMS reflect your current food safety hazards, legislation, or operational changes?

➌ Do you have evidence of continuous improvement (e.g. like the Plan-Do-Check-Act cycle)?

Finally, once that’s done, you’re ready to move on to the next phase. 

You’re ready to take Aim

2️⃣ AIM Phase

The Aim Phase is all about focusing and rehearsing before your EHO inspection.

The first step is to take aim.  This means, you’ll focus your attention on two key areas:

Structure & premises, and knowledge & behaviour (see image above).

Once attention has been focused on these key areas – you will rehearse before the EHO visit.

How To Take Aim

Follow these steps to simulate the EHO inspection effectively:

✅ Conduct a full mock EHO inspection (similar to how the EHO would inspect your premises). 

This means you must walk the premises as if you are the inspector.

Top Tips For Taking Aim

Tip #1️⃣ Invite staff from different departments to inspect each other’s areas.

Tip#2️⃣ Download our EHO inspection checklist to prepare properly.

Tip#3️⃣ Ask, “Why is this happening?” when you spot poor practice — and dig until you find the root cause of the problem. 

Once you’ve spotted the gaps, shift to action mode:

✅ Build an Action Plan to fix what’s wrong.
✅ Involve your team — delegate clearly.
✅ Share what you found in meetings or quick daily huddles.
✅ Keep the EHO inspection on top of everyone’s mind.
✅ Stimulate a work culture atmosphere where everyone is alert and ready (because EHO inspections are unannounced).

Brief Staff Before The EHO Inspection Visit

Remember — it’s important to brief staff thoroughly, and go over the essentials before the EHO visit 📢:

Explain what to do when the EHO inspector arrives.

✅ Identify who should be called when the EHO arrives unannounced.

✅ Show staff where key documents are stored (never leave these locked in an office).

✅ Prepare staff for the kinds of questions they might face — and how to respond confidently. 

✅ Highlight the key practices the EHO will be checking.

Moreover – keep this in mind:

The EHO (Environmental Health Officer) won’t just inspect your kitchen. They’ll observe behaviour and ask questions. In addition, the inspector will look for signs that your documentation is in frequent use.

This is why the Aim Phase is so powerful.  It lets you catch issues before the EHO visit happens.

Now let’s move to the final phase – The Fire Phase

3️⃣ FIRE Phase

The Fire Phase is all about executing confidently on EHO inspection day. 

When this day finally arrives, here’s how to stay in control:

✅ Focus your team — get everyone aligned with the same mindset (like before an interview or a driving test).

✅ Remain calm, professional, and cooperative.
✅ Provide requested documents quickly and with confidence.

For instance, walk the inspector through your business clearly, without sounding defensive.

At that point, your preparation needs to turn into confident execution.

So, once the EHO inspection is underway:

✅ Answer questions directly and honestly.
✅ Don’t be afraid to ask the inspector to rephrase something if you’re unsure.
✅ Show that you understand the why behind your safety controls — not just what they are.
✅ And finally, avoid guessing, speculating, or over-explaining.
