✅ Ask, “Why is this happening?” whenever you spot poor practice — and dig until you find the real cause.
Once you’ve spotted the gaps, shift to action mode:
✅ Build a plan to fix what’s wrong

✅ Share what you found in meetings or quick daily huddles
